How to Obtain Services
If you would like to receive any of the equipment above, ask your doctor for a referral to Tri-Care Home Medical or
call us and we can discuss the referral process for you. Referrals for medical equipment may be received directly
from your physician, a hospital discharge planner, a nurse, a physical therapist, or a social worker. If the item
does not require a physician's order, you may just call or come by Tri-Care Home Medical to purchase the
equipment.
